Pre-Elevation and Elevation to Dispute Resolution

Notification and Subject Review

  1. A notice of a report in the NPDB is mailed.
  2. The subject of the report reviews the report for accuracy.
  3. The report is accurate, the scenario ends.

Enter Into a Dispute Status

  1. The subject of the report enters the report into dispute status.
  2. The subject of the report takes no further action.

Prerequisites for Dispute Resolution

  1. The subject of the report must contact the reporting entity to resolve the issue.
  2. The subject of the report must wait 60 Days. If the reporting entity changes the report and the subject is satisfied with the changes, the scenario ends.
  3. If the reporting entity does not change the report or if the subject of the report is not satisfied with the changes, the subject of the report requests that the report be elevated to the Dispute Resolution and submits supporting documentation.

Dispute Resolution

  1. The dispute case is reviewed to ensure it has met the dispute and pre-elevation Dispute Resolution requirements.
  2. If the dispute case does not meet the requirements, the subject of the report must contact the reporting entity to resolve the issue.
  3. If the dispute case does meet the requirements, the dispute case is elevated to dispute resolution.
  4. The dispute case is reviewed, additional information is requested, if necessary.

Review Decision and Notification

  1. A decision is made:
    • The report is accurate as submitted.
      • A decision letter is sent to the subject of the report with a copy to the reporting entity.
      • All queriers who received notification of the dispute, receive a copy of the disputed report with a summary of the decision.
    • The report is inaccurate as submitted. Proceed to Table F-3.
    • The report is outside the scope of review.
      • A decision letter is sent to the subject of the report with a copy to the reporting entity.
      • All queriers who received notification of the dispute, receive a copy of the disputed report with a summary of the decision.
