Report Number Help
The NPDB notification letter you received in the mail is to inform you that you (or your organization) are the subject of an NPDB report. The letter includes the report number and password you need to sign in. If you lost the number or password you must order a Self-Query to access your report.
A Self-Query is a search of the NPDB for your own information and costs $3. You can access your report from the online results.
