How to Sign in to Your NPDB Report

If you are signing in to view your NPDB report for the first time, you will need information from the notification letter you received from the NPDB.

You will need to enter the report number and access code from your letter. If you do not have the report number from the letter, you must request a Self-Query to access your report.

Effective December 1, 2023, the NPDB requires the use of multi-factor authentication, or MFA, to sign in to our system. The NPDB has partnered with ID.me to provide this service.

To Sign in to Your Report

Note: Only the subject of the report can sign in to their report.

  1. On the home page, select Sign In to View Your Report.
  2. On the Sign In To Review Your Report page, select Sign in.
    • If you already have an account with ID.me, sign in to your account. Otherwise, select Create an ID.me account.
    • On the final step of creating your ID.me account, select the option to allow access to XMS. This is required to connect your NPDB account to your ID.me account.
  3. On the NPDB Account ID page, enter your account ID, then select Continue. Your account ID may be the report number from your notification letter.
  4. Next, enter your NPDB password or access code. If you have signed in to your report in the past, enter the password you set for your account. Otherwise, enter the password or access code from your notification letter.
  5. If you are signing in to your own report you must verify that you are the subject of the report. On the Subject of Report page, select Yes, and then Continue.
  6. If the system displays a Practitioner Information page, enter the requested information, such as your date of birth or Social Security number, and then Continue.
  7. If the system displays an Account Information page, review your contact information and update it. Select Submit.

The next time you sign in with ID.me you will go directly to the Your Reports page.
