What is a report disclosure history?

Your report disclosure history is a list of entities that have received your NPDB report(s) as part of a query response. It shows when and to whom your report information was disclosed, but does not include the details of the reports themselves.

This feature is only available with an active subscription. If you do not have any NPDB reports that match your information, a disclosure history will not be displayed.

Your history is updated whenever your report(s) are disclosed to an entity or when your reports change.

The report disclosure details show:

  • The entity's contact information
  • The date(s) they accessed your information
  • The type of query they submitted

Query types:

  • Query Response: A one-time snapshot of your reports at a specific point in time
  • Enrollment: Ongoing monitoring where the entity is automatically notified of changes to your reports

To access your report disclosure history:

  1. Select Sign In on the top right of the NPDB home page, then select Personal Account.
  2. On the Self-Query/View Your Report Sign In page, select Sign in.
  3. Sign in using your ID.me email and password.
  4. Select Your Subscription in your personal account.
  5. Select Review your report disclosure history.
  6. To view details, select an entity name.
  7. To download your history, select Download Disclosure History to export an Excel file.
