Attestation is our national education and outreach effort to ensure that all registered organizations are meeting their NPDB reporting requirements. Currently, State Licensing and Certification Agencies (State licensing boards), health centers, and hospitals complete attestation.

As part of the registration renewal process, organizations will be prompted every two years to complete the attestation process. The NPDB will send a 60-day reminder email to the organization's Data Bank administrator prior to the NPDB registration renewal/attestation due date. If your organization has not received notification, then your renewal is not currently due. Detailed instructions for attestation will be available when you sign in to your NPDB account to renew.

To Attest

  1. Sign into the NPDB as the Data Bank administrator and renew your organization's registration.
  2. After completing the renewal, select Continue to Attestation.
  3. You may be prompted to answer some questions about your organization. Complete the information and Continue to the Next Step.
  4. The Attestation section shows a link to a summary of your organization's reporting history and the legal requirements for reporting to the NPDB. You may attest to whether your organization is in compliance with NPDB reporting requirements.
  5. Finally, review your information on the Certify and Submit section. If everything is correct, select Submit. If you need to correct information before you submit, select the Edit button for the section you need to correct.
