Attestation is our national education and outreach effort to ensure that all registered organizations are meeting their NPDB reporting, querying, and confidentiality requirements.

As part of the registration renewal process, organizations are prompted to complete the attestation process every 2 years. The NPDB sends a 60-day reminder email to the organization's Data Bank administrator(s) prior to the NPDB registration renewal due date. If your organization has not received notification, then your renewal is not currently due.

All information submitted to the NPDB is considered confidential. This information may not be disclosed except as specified in the NPDB statutes and regulations. The confidential receipt, storage, and disclosure of information are essential to NPDB operations.

To Attest

  1. Sign in to your Data Bank administrator account and renew your organization's registration.
  2. After completing the renewal, select Continue to Attestation.
  3. You may be prompted to answer some questions about your organization. Complete the information and Continue.
  4. The Attestation section shows a link to a summary of your organization's reporting and querying history and the legal requirements for reporting to and querying the NPDB. You should attest to whether your organization is in compliance with NPDB requirements.
  5. Finally, review your information on the Certify and Submit section. If everything is correct, select Submit. If you need to correct information before you submit, select the Edit button for the section you need to correct.

To Edit Attestation

  1. Sign in to your Data Bank administrator account.
  2. Select Attestation on the Select an Option page.
  3. Select Edit Attestation on the Attestation Confirmation page.
  4. Edit the information under Are You Authorized to Attest? as needed. If you select the box next to "I am authorized to attest," the system will populate your information.
  5. Select Yes or No in response to the attestation question. If you select No, a reason is required. Select Continue.
  6. Select Submit.
  7. You will receive confirmation that the attestation is complete. You can select View Printable Copy to view a PDF of the confirmation.

Additional Steps

  • If your organization is a health center , you will be able to edit, add, and remove your locations during this process.
  • If your organization is a hospital , you will be able to edit your clinical privileges professions during this process.

To Edit Attestation as a State Licensing Board

  1. Sign in to your Data Bank administrator account.
  2. Select Compliance on the Select an Option page.
  3. Select Attestation on the Compliance page.
  4. Select Edit Attestation on the Attestation Confirmation page.
  5. Select Yes, I'm ready to start now.
  6. Verify your regulated professions by adding, removing, or editing the list as needed, and then select Continue.
  7. Attest to your reporting compliance and edit selections as needed. Confirm the certification information. Select Continue to Compliance Review Summary.
  8. You will receive a confirmation message confirming your submission.
  9. Select Attestation on the Compliance page and then select View Printable Copy to view a PDF confirmation.
