I received a letter in the mail about my organization. How do I view the report?
Complete the following steps to view an organizational report:
- Go to the Sign In page.
- Select the checkbox I affirm I am an executive or owner of my organization on the right side of the page.
- Select View a report. You may need to provide information from a notification letter you received from the NPDB.
- Sign in with ID.me and complete identity verification.
- Enter the Account ID and access code from the notification letter if required.
- Select the checkbox to acknowledge and agree to the Rules of Behavior for the NPDB and select Continue.
- Review the Account Information, enter your information in the certification, and select Submit.
- View the report on the Your Reports page.
