I entered a subject into my subject database; why am I not able to query them?

You will not be able to query a subject with incomplete information. Please review the subject's information in the subject database to ensure it is complete. If the subject's information is complete, a Y will appear next to their name.

If the subject's information is not complete, complete the following:

  1. Select the name of the subject, then select Edit, and verify the following:
    • First and last name
    • Date of birth
    • Gender
    • The subject's social security number (SSN) or individual taxpayer identification number (ITIN) is accurate and complete
    • The professional school and year of graduation is entered
    • The field of licensure, license number, and state are entered
    Note: If you have entered the National Provider Identifier (NPI) number for your subject, verify that you entered the correct NPI number on the NPI website. If the number is not correct, your subject may show as incomplete.
  2. Select Validate Without Storing, located at the bottom of the form, to ensure that the subject's information is valid and complete. Lastly, select Store to complete the save of your subject's updated information.
