I am under 18. How do I submit a Self-Query?
If you are under 18, you must call the Customer Service Center to request a Self-Query. We will ask for your personal information, including the following:
- Your Social Security number
- The healthcare profession for your current or future license
- The state issuing your license
- Your license number (if you have one)
- The name of your school or training program
- The year you graduated or completed training (or the year you will graduate or complete training)
- Payment details to process the Self-Query (card number, expiration date, and cardholder name)
Your Self-Query will take 6-8 weeks to process. We will mail you a form using the information you provide. Your address must be in the U.S. (including U.S. territories and military addresses).
You must notarize the form and send it back. Once we receive it, we will process your request and charge your card. Your Self-Query response will be mailed to you via U.S. Certified First-Class Mail. A signature is required upon delivery for all mailed paper Self-Queries. Do not open the envelope before giving it to your licensing or certifying organization.
