Hours of operation for the NPDB Customer Service Center

Monday - Thursday

8:30 a.m. - 6:00 p.m.

Friday

8:30 a.m. - 5:30 p.m.

Closed on weekends and Federal holidays. Hours of operation are in Eastern Time.

Note: Do not include Personally Identifiable Information (PII) in any email or email attachment to the NPDB. We require users to securely upload documents after logging in with their NPDB credentials. If you do not have the option to upload documents, please call us. Additionally, do not send any Protected Health Information (PHI). PHI is not required for NPDB reporting, querying, or the dispute process.



Mailing Addresses

Almost all documents can be submitted to the NPDB through a secure, electronic submission. For faster processing, please call us before mailing any documents such as dispute materials so we may assist you with this process.
