What to Do After Receiving an NPDB Report Notification
If you received a notification letter from the National Practitioner Data Bank (NPDB), it means you are the subject of a report in the NPDB and have information available to review. The NPDB is a federal repository that collects and maintains information on health care practitioners and organizations. If a report has been submitted about you or your organization, you will be notified so you can review the report and ensure the information is accurate. You can also respond to the report.
For Individual Health Care Professionals:
Note: Only the subject of the report can sign in to their report.
- Select Sign In.
- Sign in using your ID.me credentials.
- After completing your profile, select Your Reports to view your report(s). An Account ID is not required.
For Organizations:
Note: Only the executive or owner of the organization can sign in to their report.
- Go to the Sign In page
- Select the checkbox I affirm I am an executive or owner of my organization on the right side of the page.
- Select View a report.
- Sign in using your ID.me credentials.
- Enter the account ID and access code from the notification letter received.
- Select the report number to view the full report.

Sign in to View Your Report with an identity-verified ID.me account.
