What do I do if I am unable to sign for my certified mail Self-Query?

All mailed paper Self-Queries are sent via U.S. Certified First-Class Mail due to their sensitive nature. A signature is required upon delivery to ensure your information is kept secure. All mailed paper Self-Queries require a signature. Do not open the envelope before giving it to your licensing or certifying organization.

If you are unable to sign for your mailed paper Self-Query, USPS will leave a notice in your mailbox. Take the notice to the post office to get your Self-Query. You can also ask your licensing or certifying organization if they will accept the digitally certified Self-Query instead of a paper Self-Query.
