How do I download my certified Self-Query response?

You can view and download a certified Self-Query response at any time with an active subscription. Complete your profile within your personal account.

To download the certified Self-Query response file:

1. Select Your Subscription in your personal account.
2. Select the Get your self-query response button link or the View Certified Self-Query Response button.
3. Select the blue Save button on the top of the Self-Query Response window.
4. Save the PDF file to your device.
5. Use Adobe Acrobat to view the PDF file.

The PDF response file is your personal file to do with as you wish. The NPDB does not email Self-Query responses and does not send responses to third parties, such as state licensing boards.
