Can you mail or email my Self-Query results directly to a state licensing board?
No. For security reasons, we can't mail or email your Self-Query response to any third party. It must be mailed to you at a U.S. address (including U.S. territories and military addresses). A signature is required for all mailed paper Self-Queries. Do not open the envelope before giving it to your state board.
