What if a licensing board is asking for paper Self-Query?
The NPDB encourages all state licensing boards to query the NPDB directly. However, if a Self-Query is required from applicants, boards are encouraged to accept the digitally certified Self-Query PDF. A certified Self-Query response provides assurance that the document is unaltered and displays the exact information that is contained within the NPDB. Certified Self-Queries:
- Have faster response times with no extra costs required for mail
- Do not require physical processing or storage
- Eliminate the need for a signature upon delivery
In the event that a state licensing board requires a U.S. Certified First-Class mailed paper copy of your Self-Query response, you must request a paper copy be mailed to your home or work address. Mailed paper copies of Self-Queries are an additional $13.00 each. Do not open the envelope before giving it to your licensing or certifying official.
