How to Deactivate an Authorized Agent

Both health care organizations and their designated agents have the ability to end an authorized agent relationship. The administrator for either organization can terminate the relationship using the appropriate process provided below. Once an administrator deactivates the relationship, an email notification is sent to the other organization's administrator requesting that the receiving organization accept the deactivation.

To Deactivate an Authorized Agent Relationship

For Health Care Organizations

A health care organization's administrator must follow these steps:

  1. Sign into the NPDB as the administrator.
  2. On the Select an Option page, select Administrator Options.
  3. On the Administrator Options page, select Maintain Agent Information; a list of your Agent Relationships appears.
  4. Select a relationship to deactivate and select Deactivate.
  5. On the Certify Agent Deactivation page, confirm that the correct agent is selected, certify the deactivation, and select Continue; a formatted copy of the Agent Relationship Deactivation Notice displays.
  6. Print or save a copy of the notice for your records.

For Authorized Agents

An agent's administrator must follow these steps:

  1. Sign into the NPDB.
  2. On the Select an Option page, select Administrator Options.
  3. On the Administrator Options page, select Maintain Entity Relationships; a list of the health care organizations that you represent appears.
  4. Select a relationship to deactivate and select Deactivate Relationship.
  5. On the Certify Entity Deactivation page, confirm that the correct health care organization has been selected, certify the deactivation, and select Continue; a formatted copy of the Entity Relationship Deactivation Notice displays.
  6. Print or save a copy of the notice for your records.
