How do I send my certified Self-Query response to a state licensing board?
If you have an active subscription, you can access and download your certified Self-Query response at any time from your personal account.
To get your Self-Query response:
- Sign in to your personal account.
- Select Your Subscription.
- Select Get your Self-Query response.
- Download and save the response to your device.
Once downloaded, the Self-Query response is your personal file. You can send it to a state licensing board or other entity according to their instructions.
The NPDB does not email your Self-Query response to you or send it directly to third parties, such as state licensing boards.
The blue signature bar with certifying signature is only visible when the document is opened in Adobe Acrobat Reader.
