How do I review my report?

You will need information from your notification letter.
You will need to enter the report number and access code found on the notification letter. If you do not have the letter, you can request a Self-Query to access your information.
Sign in to ID.me or create an account.
The NPDB partners with ID.me to provide secure access to your report with multi-factor authentication or MFA. If you have an ID.me account you can use it to access your report. Otherwise, select Create an ID.me account on the ID.me sign in page. On the final step of creating your ID.me account, select the option to allow access to XMS. This is required to connect your NPDB account to your ID.me account. Learn more about MFA.
You may be required to provide additional information.
For security, you may be required to enter your Social Security number, date of birth, or both.
After you sign in, verify your contact information.
Review, verify, and update your contact information as needed. After you verify your contact information you can view your report.
