How to Sign in and Review Your NPDB Report
If NPDB notified you of a report, follow the instructions below.
Health Care Professional Report
Note: Only the subject of the report can sign in to their report.
- Select Sign in on the personal account page.
- Sign in with ID.me and complete identity verification.
- Activate your personal account by reviewing and accepting the terms in the Rules of Behavior and Subscriber Agreement.
- Select Complete your profile to search the NPDB for reports that match the information you provide.
- Once complete, select Your Reports on the personal account home page.
- A summary of your report is displayed. Select the report number to view the full report.
Additional steps: Subscribe to get your report disclosure history. With an annual subscription you can access a list of entities that received your report(s), if any, in a query response.
Organizational Report
Viewing your report for the first time requires information from the notification letter.
Complete the following steps to view an organizational report:
- Go to the Sign In page.
- Select the checkbox I affirm I am an executive or owner of my organization on the right side of the page.
- Select View a report. You may need to provide information from a notification letter you received from the NPDB.
- Sign in with ID.me and complete identity verification.
- Enter the Account ID on the Account ID page.
- Select the checkbox to acknowledge and agree to the Rules of Behavior for the NPDB and select Continue.
- Review the Account Information and update it if needed.
- A summary of your report is displayed. Select the report number to view the full report.
