How to Verify and Update Your Address

The NPDB notifies the subject of a report, at the address provided by the reporting entity listed in Section A of the report. If you receive a report with an incorrect address, you may update the home or work address (or both) by signing into the report online.

To Update Your Address on a Report

  1. On the NPDB homepage, select Check Your Report.
  2. On the Sign In with a Report Number page, enter the report number and password, then select Sign In.
    Note: If you are signing into your report for the first time, use the report number and password listed on the notification letter to sign in.
  3. Read the information on the Rules of Behavior page, check the box to acknowledge the rules, and then select Continue.
  4. On the Practitioner Identification page, enter your date of birth and/or your SSN/ITIN, then select Continue.
  5. On the Report Response Options page, select Update your password, email options, or mailing address.
  6. On the Update Options page, select Update Mailing Address.
  7. On the Change Address page, you may correct or update your home and/or work address.
  8. Complete the Certification Data section (your name, title and phone number) and submit. The Change of Address Confirmation page will display.
