How to Verify and Update Your Address
We notify the subject of a report at the address the reporting organization provided. If you receive a report with an incorrect address, you should update it by signing into the report online.
To Update Your Address on a Report
- On the home page, select Sign in to View Your Report.
- On the Sign In with a Report Number page, enter the report number and password, then select Sign In.
Note: If you are signing into your report for the first time, use the report number and password on the notification letter. - Read the information on the Rules of Behavior page, check the box to acknowledge the rules, and select Continue.
- On the Practitioner Identification page, enter your date of birth and your SSN or ITIN, then select Continue.
- On the Report Response Options page, select Update your password, email options, or mailing address.
- On the Account Profile page, you may correct or update your home and work addresses.
- Complete the Certification section and select Submit.
