Individual One-Time Query and Continuous Query are Merging into - NPDB Query

On December 4, 2026, the NPDB will release NPDB Query, merging the Individual One-Time Query and Continuous Query services. Over the coming months, the NPDB will add system enhancements to the Integrated Querying and Reporting System (IQRS) and the Querying and Reporting XML Service (QRXS) to support NPDB Query and improve usability. NPDB Query will incorporate the best of both query services. Continue to monitor this page for updates.

Timeline to NPDB Query

March 13, 2026

  • Organizations can now export a file of Individual One-Time query subjects and import that file into Continuous Query as new enrollments. A validation report is available to identify any invalid, incomplete, or duplicate subjects in the import file that need correction.

July 2026

  • For each enrollment, organizations will have the flexibility to identify the enrollment duration: from one-day to 12-months, or any date in between. Organizations can also choose to end the enrollment immediately after the initial response is complete.
  • Organizations will be able to resume a canceled enrollment within the enrollment period in IQRS and also through QRXS. The enrollment confirmation and billing history will reflect all enrollment periods.

December 4, 2026

  • Individual One-Time Query and Continuous Query will be merged into NPDB Query.

How to Prepare for NPDB Query

If you are already using Continuous Query:

  • Your enrollments will transfer to NPDB Query enrollments. There is no action to take.

If you are using One-Time query:

  • Create a plan to begin using Continuous Query or start using NPDB Query when it is released in December 2026. To help in the transition, organizations can now export a file from Individual One-Time query subjects and import that file into Continuous Query as new enrollments. In December, Continuous Query enrollments will transfer to NPDB Query enrollments.

If you are querying Organizations:

  • You will still maintain the ability to query organizations. NPDB Query will only affect Individual One-Time Query and Continuous Query. Upon the release of NPDB Query, organizations will start an Organization Query from the Query Options menu.

If you are using a third-party credentialing system:

  • If your in-house or third-party credentialing system sends Individual One-Time Query transactions through QRXS, we strongly encourage you to implement Continuous Query. Continuous Query enrollments will transfer to NPDB Query enrollments. Use your Vendor ID on the Software Sign in page to access the Continuous Query Specifications. NPDB Query will be the only query transaction that can be used when it is released in December 2026.

  1. Why is the NPDB merging Individual One-Time and Continuous Query?
    Merging Individual One-Time and Continuous Query services will simplify the query process and make the system easier to use. Each query will provide an immediate snapshot response and ensure that querying organizations are notified if new adverse information is reported. This approach advances the NPDB’s mission to promote health care quality, protect the public, and reduce health care fraud and abuse in the United States.

  2. I already use Continuous Query. What do I need to do?
    Good news! If you are already using Continuous Query, there is no immediate action to take. Your Continuous Query enrollments will transfer to NPDB Query enrollments in December 2026.

  3. I use Individual One-Time Query. When do I need start using NPDB Query?
    When NPDB Query is released in December 2026, it will be the only Query service available. Don't wait! We strongly encourage you to transition to Continuous Query. Continuous Query enrollments will transfer to NPDB Query enrollments when NPDB Query is released. In addition, by using Continuous Query, you can take advantage of upcoming system enhancements. To help in the transition to Continuous Query, organizations can now export a file from Individual One-Time query subjects and import that file into Continuous Query to create new enrollments.

  4. How can I enroll many providers all at once into Continuous Query?
    You can Import an Excel File to submit a bulk Continuous Query enrollment. To make enrolling easier, you can also Export a File of Individual One-Time Query subjects and Import the file to create new Continuous Query enrollments. Your Continuous Query enrollments will transfer to NPDB Query enrollments when NPDB Query is released in December 2026.

  5. I use a third-party credentialing system vendor. Will they be notified to transition to NPDB Query?
    Individuals on the QRXS mailing list will be notified of the QRXS enhancements that will affect their software implementation in the transition to NPDB Query. Organizations and system vendors can sign up for the QRXS mailing list on our Software Sign in page. We also recommend contacting your system vendor to discuss their plans to implement Continuous Query early or move to NPDB Query when it is released in December. The NPDB does not directly support credentialing/third party software. Any vendor relationships are outside the purview of NPDB.

  6. I query organizations using QRXS. Will there be a change for me?
    Organizations can still query organizations through QRXS when NPDB Query is released. However, if you also use QRXS for Individual One-Time Query transactions, you will need to transition to Continuous Query early or to NPDB Query when it is released in December 2026. Your Continuous Query enrollments will transfer to NPDB Query enrollments.

  7. I only query my providers every 2 or 3 years. Because you must renew enrollments annually, wouldn't NPDB Query increase my querying fees?
    NPDB Query retains the best of both query services. Organizations can enroll providers for a period of up to one year as with current Continuous Query functionality. If the organization chooses not to maintain the enrollment, it can end the enrollment immediately after the query response is complete, mirroring current One-Time Query functionality. The organization would then create new query enrollments during its next credentialing cycle. The query fees would remain the same.

  8. What will be the fee for NPDB Query?
    The annual enrollment fee for NPDB Query will be $2.50 which covers the cost of enrolling one practitioner in the NPDB for up to one year.
